Medication Support :
RK's Mother Teresa Health Care At Home will work with you to make sure that you take your medication on time, every day. We dispense and deliver medication and subsidiary items to patients or their care takers at work or at home. We also provide treatments at times and places of their choice.

Key service delivery elements include:

Prescription management with the referred center to ensure the patient always has the right dose of medication available at the right time

Monitoring the stock levels as a mark of adherence to the prescribed medication regime and to ensure efficient use of high value medications

Regular contact with patients to ensure medication is delivered as required and that patient contact details are kept up to date


The support provided to patients include:

Nurse visits to educate a patient and / or carer about the condition for which treatment has been prescribed: the correct use and storage of a medication, the management of side effects, what to do in the event of an adverse reaction etc

Patient and/or carer training in the correct use of a medication-dispensing device, such as an injector pen or a portable infusion pump Visits from our trained nurses to complete tests between treatment cycles and provide ongoing clinical support

Active remote support, such as a program of telephone calls to educate, deliver messages and to remind a patient what medication to take and when


Medication Home Treatment :
RK's Mother Teresa Health Care At Home provides complex and highly sophisticated clinical services inside the comfortable and secure setting of a patient's home. Our medication home treatment services range from oral oncology to enzyme replacement therapy. We focus our service to give patients the flexibility and freedom to simply get on with their lives. Our professional nursing staff is always at your disposal to help you in fulfilling your medical requirements. Our nurses will train you how to manage your health condition and also in the administration of medication. We are equipped with specialist IV nurses, oncology, hemophilia and pediatric nurses to help you out. Nurses are trained on an annual basis in conjunction with mandatory requirements including CPR, POVA and infection control.

The medications in question typically have one or more of the following characteristics:

Are given by injection or infusion
Are difficult or should not be self-administered
May require careful preparation ahead of administration
May have special transport and storage requirements

End of Life Care :

Our end of life care service prevents hospital admissions by supporting and caring for people at home in the last 12 months of life. End of life care refers to health care, not only for patients in the final hours or days of their lives, but more broadly care of all those with terminal illnesses or treatment condition that has become advanced, progressive and incurable. We provide patients with drug access support, clinical triage and a rapid response service for 24 hours a day, 365 days a year. Patients are assigned a fully qualified, specialist nurse who provide personalized care and organize care plans to suit their individual needs.
